Tesamorelin is a synthetic GHRH(1-44) analogue with a trans-3-hexenoic acid modification at the N-terminus. It binds GHRH receptors on pituitary somatotrophs, stimulating endogenous GH secretion via cAMP/PKA signaling while preserving physiological GH pulsatility. FDA-approved (Egrifta) for HIV-associated lipodystrophy. Peak GH levels at 15-45 minutes post-injection.
